NTSEKENDIO Paul NYIBIO ;\u00a0Data Manager: M. Herv\u00e9 TCHOKOMENI<\/p><p><strong>Summary:<\/strong><\/p><p>Cameroon is facing COVID-19 outbreak since March 6, 2020. In November 18, 2020, the 56<sup>th<\/sup>\u00a0<em>SitRep<\/em>\u00a0reported 23,528 confirmed cases, 435 deaths (CFR=1.8%), 169\/190 (88.9%) districts affected and 916 active cases over the national territory.\u00a0 Littoral, including Douala, is one of the most affected and vulnerable regions of the country in terms of confirmed cases and deaths<sup>1,<\/sup>. It is known to carry the highest burden of health personnel (553 affected) and pregnant women affected (72 affected). Despite the interventions implemented by the Cameroon Ministry of health and partners, the spread of the disease continues.<\/p><p>REMMOCC project is implemented in Cameroon since January 2021 with the aim of reducing the mortality and morbidity of COVID 19 in Douala through health facility and community based activities. The projects\u2019 activities target the most affected health districts of Douala in terms of COVID 19. In order to reach the objective, community health volunteers were trained to sensitize households on COVID 19 contamination and prevention, and to identify suspected cases in households. They implemented household sensitization and surveillance of suspected cases from March to September 2021. Case detection was also done in each health facility by dedicated focal points through registers\u2019 review. Identified suspected cases were tested, linked to care and their contacts identified for potential positive cases detection. Prevention of Mother-to-Child Transmission (PMTCT) of HIV is among the key HIV prevention strategies set by the Cameroon National AIDS Control Committee (NACC) to fight HIV\/AIDS. Despite efforts being done in the implementation of this strategy, PMTCT implementation remains suboptimal especially in this region endowed with numerous restrictive factors such as: spontaneous terrorist attacks, poor access to healthcare services, low income status, harsh climatic conditions, limited access to electricity, water and sanitation, and bad road network.<\/p><p>M.A.SANTE implemented the AT-PMEL project from April 2018 to April 2020 in Mada and Kousseri Health Districts situated in the lake Chad Basin in the Far North region of Cameroon. HIV screening sessions (targeting mainly pregnant women with the aim to refer them to health facilities for better healthcare) were organised in collaboration with traditional birth attendants and health facilities. PMTCT activities were done in health facilities and communities of Mada and Kousseri Health Districts. Compliance with hygiene measures and vaccination are among other measures to fight against the disease. Oral cholera vaccine (OCV) is now being used widely to prevent cholera in areas at risk. The standard dosing schedule of OCV is two doses with the second dose given 2 weeks after the first dose. This project was conceived and implemented to assess whether the immune response of the second dose of oral cholera vaccine (OCV) administered at extended intervals (2 weeks, 06 months and 11 months) is equivalent to that of the manufacturer which is two weeks after the first dose. After obtaining administrative authorizations and ethical clearance consent participants were recruited. Vaccination and blood collection were done during a year and a half at the Medical Center of Soboum located in the Littoral region of Cameroon. Serum samples were analyzed at M.A. SANTE Laboratory to determine the vibriocidal response at different periods of time. This project is funded by EDCTP (\u00ab\u00a0European&amp;Developing Countries Clinical Trial Partnership\u00a0\u00bb) and is implemented by the Division of Operational Health Research, the Directorate of Pharmacy, Medicines and Laboratories of the Ministry of Public Health of Cameroon, the Research Ethics Committee and the NGO M.A. SANTE (Better Access to Health Care). The prerequisite for the above-mentioned training is the conduct of a needs assessment study on training in research ethics and pharmacovigilance by M.A. SANTE. It is expected that this study will identify and prioritise training needs and targets in terms of the protection of research participants so that these can be used in the programming and design of the training modules planned \u00a0within the framework of the BREEDSAFCA project.<\/p><p><strong>RESEARCH QUESTION<\/strong><\/p><p>What are the different training needs in ethics for the protection of research participants and in pharmacovigilance in Cameroon<\/p><p><strong>OBJECTIVES<\/strong><\/p><p>Assess training needs for the protection of research participants and in pharmacovigilance in Cameroon.<\/p><p><strong>Methodology<\/strong><\/p><p>It was a descriptive cross-cutting study that collected data on one hand on the training needs in research ethics from members of Human Health Research Ethics Committees, researchers, and training institutes; and on the other hand on the training needs in pharmacovigilance for health personnel, pharmacists, health training and health programmes in Cameroon using specific questionnaires pre-tested and administered face-to-face. The data was collected during the period of June 2020 by trained investigators.<\/p><p><strong>\u00a0<\/strong><strong>ACTIVITIES<\/strong><\/p><p>The activities that are carried out so far are :<\/p><p>\u2013 Development of the protocol and data collection tools ;<\/p><p>\u2013 Pretext of the data collection tools ;<\/p><p>\u2013 Submission of the Protocol to the National Research Ethics Committee;<\/p><p>\u2013 Submission of the protocol to the different target institutions of the project for obtaining administrative authorisations;<\/p><p>\u2013 Development of the digital tools for the survey;<\/p><p>\u2013 Training of supervisors and investigators;<\/p><p>\u2013 Data collection;<\/p><p>\u2013 Data processing;<\/p><p>Future activities to be carried out are data analysis, report writing and dissemination of results to project partners. Walter (Data manager)<\/p><p><strong>Project description<\/strong><\/p><p>The current perception of cholera in Africa is that outbreaks occur suddenly and unexpectedly. To verify this hypothesis in the Cameroon context, this project was implemented to demonstrate the established seasonality of Cameroon. We conducted a cholera surveillance system in the Far North region of Cameroon using innovative epidemiological and laboratory methods which are sustainable for resource limited settings. This project was conducted from 2013 in the Far North region of Cameroon as it was the region with the most reported cases of cholera and presented many at risks factor such as being bordered by the Lake Chad. Prior to the study, administrative authorizations were obtained, the protocol developed and approved by the national ethics committee in human health research. Six health facilities from the health districts of Kousseri, Mada, Makary and Goulfey were selected at sentinel sites. In the study sites, study nurses were trained to enroll by collecting diarrhea samples of study participants who visited health facilities for treatment of severe watery diarrhea and dehydration. Combined to the latter, environmental samples of surface water sources were collected periodically to ensure joint environmental surveillance. Dans le but de contribuer \u00e0 la r\u00e9duction de la mortalit\u00e9 et la morbidit\u00e9 due aux maladies diarrh\u00e9iques, nous proposons d\u2019\u00e9valuer la relation entre les comportements des populations li\u00e9s \u00e0 l\u2019eau et la survenue des cas de diarrh\u00e9e dans le m\u00e9nage.<\/p><p><strong>Mat\u00e9riels et m\u00e9thodes :\u00a0<\/strong>Une \u00e9tude transversale descriptive suivant un \u00e9chantillonnage al\u00e9atoire \u00e0 plusieurs niveaux a \u00e9t\u00e9 men\u00e9e du 1er D\u00e9cembre 2013 au 2 F\u00e9vrier 2014 dans les Districts de Sant\u00e9 de Goulfey, Mada et Makary. Les donn\u00e9es ont \u00e9t\u00e9 collect\u00e9es par administration en face \u00e0 face d\u2019un questionnaire structur\u00e9 dans les m\u00e9nages et par une grille d\u2019observation pour les points d\u2019eau.<\/p><p><strong>R\u00e9sultats :\u00a0<\/strong>Quatre-vingt communaut\u00e9s issues de 17 Aires de Sant\u00e9 sur 26 ont \u00e9t\u00e9 incluses. Parmi les 531 familles visit\u00e9es, 527 (99,3%) ont \u00e9t\u00e9 interview\u00e9es couvrant ainsi une population de 4786 personnes avec 1408 enfants de moins de 5 ans. Nous avons recens\u00e9 288 points d\u2019eau artificiels et 29,5% \u00e9taient non fonctionnels. Parmi les points d\u2019eaux fonctionnels, 30,5% (60) ont \u00e9t\u00e9 class\u00e9s comme am\u00e9lior\u00e9s, et probablement non contamin\u00e9s. La plupart des m\u00e9nages (66,2%) utilisaient principalement le forage comme source d\u2019eau et les eaux de surface \u00e9taient aussi utilis\u00e9es dans 6% des m\u00e9nages. Vingt-quatre soit 4,5% des m\u00e9nages \u00e9taient connect\u00e9s \u00e0 un r\u00e9seau de distribution CDE, 7% (37) des foyers avaient un bon acc\u00e8s g\u00e9ographique \u00e0 une source d\u2019eau potable. Des comportements inad\u00e9quats des m\u00e9nages ont \u00e9t\u00e9 recens\u00e9s lors du choix des sources d\u2019eau (21,5%) ; lors de la collecte et du transport de l\u2019eau (42,7%) ;lors du stockage de l\u2019eau (81,5%) et lors de l\u2019utilisation (73,8%).Nous avons not\u00e9 une pr\u00e9valence de la morbidit\u00e9 diarrh\u00e9ique de 13,3% en g\u00e9n\u00e9rale et de 25,9% chez les enfants de moins de 5 ans pendant les deux semaines pr\u00e9c\u00e9dant l\u2019enqu\u00eate. Le type de source d\u2019eau utilis\u00e9; le r\u00f4le des enfants dans la collecte et le transport de l\u2019eau \u00e9taient les principaux d\u00e9terminants comportementaux li\u00e9s \u00e0 l\u2019eau statistiquement associ\u00e9s \u00e0 l\u2019incidence d\u2019au moins un cas de diarrh\u00e9e avec respectivement les rapports de c\u00f4tes de 2,09 et 2,43 et des valeurs-P de 0,002 et 0,001.<\/p><p><strong>Conclusion :\u00a0<\/strong>Dans le bassin du Lac Tchad, l\u2019acc\u00e8s \u00e0 l\u2019eau est limit\u00e9 et in\u00e9galement r\u00e9partie. Les habitudes inad\u00e9quates li\u00e9es \u00e0 l\u2019eau contribuent \u00e0 augmenter la fr\u00e9quence des diarrh\u00e9es. Au Cameroun, elle n\u2019est pas accessible aux populations de tous les niveaux de la pyramide sanitaire \u00e0 cause de l\u2019absence de strat\u00e9gie de surveillance ad\u00e9quate. L\u2019objectif de cette \u00e9tude \u00e9tait d\u2019\u00e9valuer la faisabilit\u00e9 de la mise en place d\u2019une surveillance \u00e9pid\u00e9miologique continue des cancers du col de l\u2019ut\u00e9rus dans le District de Sant\u00e9 de Dschang.<\/p><p><strong>M\u00e9thodes\u00a0<\/strong>: Il s\u2019agissait d\u2019une \u00e9tude transversale, descriptive et analytique qui a impliqu\u00e9e pendant quatre mois l\u2019\u00e9valuation de la disponibilit\u00e9 des ressources et des Connaissances et Attitudes Pratiques (CAP) du personnel soignant du District de Sant\u00e9 de Dschang sur les cancers du col. Une surveillance \u00e9pid\u00e9miologique de trois mois du cancer du col a \u00e9t\u00e9 mise en place chez les femmes \u00e2g\u00e9es de 20 \u00e0 65 ans \u00e0 l\u2019H\u00f4pital de District de Dschang (HDD) par l\u2019inspection visuelle du col de l\u2019ut\u00e9rus et le frottis cervical.<\/p><p><strong>R\u00e9sultats\u00a0<\/strong>: Quatre-vingt personnels de sant\u00e9, 30 Formations Sanitaires ainsi que 528 femmes ont \u00e9t\u00e9 incluses dans cette \u00e9tude. 38,8% du personnel de sant\u00e9 disent que le d\u00e9terminant principal du cancer du col de l\u2019ut\u00e9rus est le Virus du Papillome Humain ; 21,3% savent que la pr\u00e9vention peut se faire par la vaccination. 42(52,5%) personnels affirment qu\u2019i1 faut prescrire le frottis devant une femme malade et 19% personnels d\u00e9clarent l\u2019avoir d\u00e9j\u00e0 pr\u00e9lev\u00e9. Vingt huit structures de sant\u00e9 (93,3%) avaient le n\u00e9cessaire pour le pr\u00e9l\u00e8vement du frottis tandis que, 12(40%) disposaient du mat\u00e9riel n\u00e9cessaire pour la r\u00e9alisation de l\u2019Inspection Visuelle du col apr\u00e8s application de l\u2019Acide ac\u00e9tique et du Lugol Iodine (VIA\/VILI). Par rapport \u00e0 la fr\u00e9quence de femmes r\u00e9guli\u00e8rement enregistr\u00e9 \u00e0 l\u2019HDD, la surveillance \u00e9pid\u00e9miologique mise sur pied a permis de d\u00e9pister 54 fois plus de femmes. Au cours de cette p\u00e9riode, 38(7,8%) l\u00e9sions pr\u00e9canc\u00e9reuses (frottis) et 234 (48%) cas de marquage au VIA\/VILI ont \u00e9t\u00e9 r\u00e9pertori\u00e9s.<\/p><p><strong>Conclusion\u00a0<\/strong>: La mise en place d\u2019une surveillance \u00e9pid\u00e9miologique continue des cancers du col est faisable dans un H\u00f4pital de District ; pour une conduite ad\u00e9quate, il serait n\u00e9cessaire de l\u2019approvisionner en un minimum de ressource et d\u2019am\u00e9liorer la formation du personnel de sant\u00e9 sur la
